Register your kit from the comfort of your own home! Please follow these simple instructions below to get started.
1. Enter the Kit ID (this is dependent on the test and can vary in length)
2. Select the Myself or Someone Else radio button
3. Click Continue Questionnaire. A few notes:
- If the user has already completed pre-screening for this test, there should not be a “Health History"
- The health history questionnaire should not be shown if the user has completed a pre-screening step
4. Check the Agree on the consent page and click “I agree and consent”
5. Click the Next button, the arrow button under the image and the back button. Make sure that the carousel shows the different images for sample collection
6. Once all the steps are done, the button should turn into done and a “great job" prompt should be shown which will take the user to the dashboard
7. The dashboard shows the product that was just registered and it highlights that the registration step is completed. You are all finished!
